HELENE Clinic, a regenerative medicine and healthcare provider based in Tokyo, Japan, wanted to explore the Indian market’s potential for international patient acquisition. While India has a large healthcare market and growing interest in advanced medical treatments, Indian patients also have access to local stem cell providers, making international travel a significant consideration.
The initial challenge was therefore not simply generating leads. HELENE needed to understand whether there was genuine demand in India for its regenerative treatment programs, which treatment areas showed the strongest potential, what patients were searching for, and whether Indian prospects would consider travelling to Japan for treatment.
upGrowth began with a market validation pilot and then moved into a second phase focused on acquiring qualified patients. Over the engagement, the strategy evolved based on search behaviour, campaign performance, audience quality, healthcare advertising restrictions, and the growing need to differentiate HELENE from locally available treatment providers.
upGrowth managed the digital acquisition program across Google Search and Display, with ongoing strategy, optimisation, reporting and testing.

At the start of the engagement, HELENE lacked an established India acquisition funnel and needed data to determine whether Indian patients would consider travelling to Japan for regenerative treatment.
The engagement began with a market validation phase, designed to identify:

Because HELENE’s proposition is international and premium, we tested an HNWI-focused Osteoarthritis campaign using available audience signals, including higher household-income segments and selected premium behavioural signals.
The objective was to understand whether a more premium audience could generate stronger-quality engagement without excessively limiting reach.

Google Search remained the primary high-intent acquisition channel.
We also tested Display and Performance Max to evaluate incremental reach. However, broader automated campaigns produced a higher proportion of low-quality or spam inquiries, based on client feedback, so we tightened controls and focused more on high-intent Search.

The campaign generated measurable demand from Indian users and established clear indication-level search patterns, particularly around Osteoarthritis and Type 2 Diabetes.
One Indian patient who had interacted with the clinic in June subsequently travelled to Japan, visited HELENE Clinic and completed treatment. Based on client feedback, the patient was influenced by the digital campaigns and was specifically interested in anti-ageing treatment.
While this represents one patient outcome and cannot be attributed solely to paid advertising, it provided an important real-world signal that the digital activity could contribute to an international patient journey.
Search Intent Was More Valuable Than Broad Reach

Healthcare advertising restrictions limited keyword and audience expansion and contributed to a more constrained and potentially higher-cost acquisition environment.
The Real Challenge Is Patient Progression
Generating clicks and inquiries was achievable, but moving users from an initial inquiry to medical-record submission and treatment required significantly more trust and consideration.
The engagement helped HELENE move from an initial question – “Is there demand for regenerative treatment from India?” – towards a clearer understanding of where that demand exists and how Indian patients respond to an international treatment proposition.
The market demonstrated genuine interest, particularly around Osteoarthritis and Type 2 Diabetes, while search data provided valuable insights into treatment intent, geographic demand and messaging preferences.
At the same time, the engagement highlighted the complexity of international medical acquisition. Generating interest is only the first step; establishing trust, clearly communicating the value of treatment in Japan, and supporting the patient through a longer decision journey are essential to converting interest into treatment.
For HELENE, the key opportunity is therefore not simply to increase traffic, but to build a differentiated Japan-based regenerative medicine proposition supported by high-intent acquisition, stronger content, trust-building and a patient journey designed specifically for international patients.
